CAUSE
This may occur when you have modified the workflow.xml in Release 6.4 Service Pack 1 and attempt to upgrade using the modified workflow xml file.
RESOLUTION
This is a known issue which can be circumvented by replacing the modified workflow.xml with the default workflow.xml from a fresh installation.
